Apprentice Care Navigator
You will be an integral member of our care navigator team involved in a variety of tasks. The role will require the ability to work in a team but also to be disciplined and motivated whilst working on your own after appropriate training.
This is an exciting and challenging role, and the successful application will gain experience in all aspects of the role, being supported by our Reception Manager and the and management team.
Duties will include:
To provide general administrative support for the Practice, including the following:
Receive patients at reception and make appointments using the clinical system Taking phone calls and messages, referring calls to others as required (upon completion of extensive training) Respond efficiently and courteously to all queries and requests for assistance from patients Signposting patients to the correct clinician/health professional Working alongside the clinical team and NHS professionals working in external healthcare settingsTo provide a courteous and efficient reception service to patients, carers and visitors within the Practice ensuring:
Patients access the primary care services they require in a timely fashion Visitors sign in and out and the member of staff they are seeing are advised of their arrival The tidiness of the waiting room and the safety of patients in the waiting room Queries are answered or passed to an appropriate member of staff in a timely fashion Database managementTo provide general assistance and administration to the practice clinical and leadership teams, including:
Accurately updating patient records using our clinical system Acting as requested by the clinical team Phoning patients to arrange/rearrange appointments and/or confirm test results. Supporting the provision of a seamless co-ordinated service through good communication and any other tasks/duties as requested by the Partners, Practice Manager or Reception ManagerTraining:
Customer Service Practitioner Level 2.

We are a friendly practice located near the Norfolk Broads offering services to approximately 10,000 permanent residents, and the many people that come to the central Broads area during the summer months. We have six GP's (4 male and 2 female) and 4 Nurse Practitioners as well as Practice Nurses and Healthcare Assistants. We are a teaching practice and offer training for Registrars and Medical Students and are also registered to undertake research activities. We are constantly looking at ways to improve our performance so that we offer the best service possible to our patients. We have an active Patient Participation Group who helps us to review and develop our services.
Working Hours :
Monday to Friday - 37.5 hours per week, working hours TBC
