Forge strong links with GP Practices, local voluntary and community organisations, utilising their established networks.
Be a friendly source of information about wellbeing and prevention approaches.
Work with people, their families and carers and consider how they can all be supported.
Support local partners and commissioners to develop new groups and services where needed.
Build relationships with staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing.
Work closely with GP practices within the PCN to ensure that social prescribing referral codes are inputted into the clinical system.
Where appropriate, physically introduce people to community groups, activities and statutory services. Follow up to ensure they are happy, able to engage, included and receiving good support.
Support the Social Prescribers produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
Training:As part of your apprenticeship, you will be required to attend college once per week at Sheffield College - City Campus. The team works with people over the age of 18 in the community. Most of the work in the team is to complete Care Act Assessments, incorporation the Mental Capacity Act, DoLs and court work, alongside supporting with CHC Assessments.
